By participating in the 2025 Your Rewards Program you have opportunities to lower disease risk factors and address chronic conditions such as high blood pressure or diabetes.
The Program is comprised of health and wellness activities designed to address each member’s unique lifestyle and healthcare needs. Members can receive gift cards from Walmart, Darden restaurants, CVS, or Subway when they attest to completing any of the qualifying rewardable activities. Gift card dollar amounts vary and depend on the rewardable activity.

All 2025 Your Rewards Program activities must occur between January 1, 2025, and December 31, 2025. The Program gets underway on January 1, 2025. Members have until January 31, 2026, to attest to healthcare activities completed during 2025.
Enrollment in this program is offered to all plan members. However, eligibility qualifications do apply. Eligibility is based in part on results from scientific clinical research and addresses specific medical situations or clinical conditions. Reward eligibility is substantiated by claims documentation.
